Accessibility survey prize draw: Terms & conditions

How to enter the draw

Survey respondents who wish to go into the draw to receive the prize must enter their contact details on the Accessibility survey form and submit it online, or by hard copy, telephone or in person to the Disabilities Liaison Librarian.

Only one entry per person allowed.

Conditions of entry

This survey prize draw is an initiative of the State Library Victoria, 328 Swanston Street, Melbourne, Vic, 3000 ('Library').

Entry into the prize draw means you accept these terms and conditions.

Entry is free.

Entry is open to Australian residents only. Prize draw entrants must be 15 or over at the time of completing the survey.

Employees of the Library and its agencies associated with this promotion, and their immediate families, are not eligible to enter.

No responsibility is accepted for late, lost or misdirected entries.

All entries become the property of the Library.

Any personal information entrants provide will be treated as confidential and managed in accordance with the Library's Privacy statement, which is published on our website.

The Library reserves the right to modify these terms and conditions at any time without notice.

Dates

The competition commences on Thursday 5 January 2017 at 3pm.

Entries close on Friday 3 February 2017 at 5pm.

Prize draw and notification

The prize will be drawn at State Library Victoria in early February 2017.

The winner will be notified by phone and/or email. If, despite reasonable efforts by the Library, a potential prize winner does not respond to the Library within 21 days of the date of notification, the prize will be forfeited and the Library reserves the right to select a new winner.

If the winner is unable to be contacted due to provision of incorrect details, the Library reserves the right to select a new winner.

The prize must be taken up within six months of the prize draw.

Prize

A behind-the-scenes Dome to dungeons tour of the Library, for the prize winner and four guests, tailored to individual needs as appropriate.

This prize is not transferable or exchangeable and cannot be taken as cash.

There is one prize only.
